Registration Forms
12 Year Old Waiver
-
(to play in minors) - This waiver must be submitted to the Board of Directors to request a 12 year old to play in minors. By Little League rule, all 12 year olds must play in Majors unless the Board, the parent, the District Administrator, and Little League International approves the waiver.
Volunteer Application - All local Little Leagues are required to conduct background checks on managers, coaches, board of directors members, and any other persons, volunteers or hired workers, who provide regular service to the league and/or have repetitive access to, or contact with, players or teams. Managers & Coaches Application - All applicants for managers and coaches must fill out this form to be selected to manage or coach on the field.
Safety
2007 Safety Manual Parent's Guide For the Child Protection Program Injury Report Form - This form is for TWLL use only. Managers should fill out this form whenever there is an injury to a player and turn it in the the TWLL Safety Officer in a timely manner. Accident Notification Form - Parents fill this form out when their child is injured and return it to the Safety Officer. This form is used by the Safety Officer to Report Injuries and Accidents to LL International.
